Thank you for allowing us the opportunity to serve your patients!

Please click the download button in the toolbar below to print our consult form.  Once we receive a fax we will promptly schedule the patient and include a date for your charts.  We give each patient 3 attempts to schedule, after which we will send a note back to you for your records if unable to schedule.

After the patient is seen you will receive a consult letter with Dr. Crandall’s findings along with any follow-up instructions.
